A leading transplant surgeon has accused NHS chiefs of presiding over a deeply flawed system that leaves patients waiting years for life-saving operations, according to a confidential letter obtained by the BBC.<\/p>\n

Professor Derek Manas, medical director of NHS Blood and Transplant, said NHS England had created a fragmented service where staff burnout is rife, poor performance goes unpunished, and operations are cancelled at the last minute. His comments come as figures show the number of heart and lung transplants performed annually has not risen in three decades.<\/p>\n

NHS England, which oversees the health service, insisted it was “working hard to improve these life-saving services.”<\/p>\n

A System Under Strain<\/h2>\n

While NHS Blood and Transplant coordinates organ donation and supports transplant centres, it is NHS England that commissions and funds the actual transplant procedures. In his letter, sent to NHS England and a small group of patient advocates, Professor Manas argued that this central body had created a system “with poor accountability.”<\/p>\n

He highlighted a critical “mismatch” between hospital waiting lists, the availability of donated organs, surgical capacity, and access to operating theatres. This imbalance, he warned, is “leading to last-minute cancellations [and] unpredictable utilisation of donated organs.”<\/p>\n

The consequences are stark. The UK, once a global leader in transplantation, now lags behind many Western nations. The NHS transplants just one in ten donated lungs and one in seven donated hearts\u2014half the rate seen in other countries. A lack of investment in modern technology, combined with shortages of theatre space, intensive care beds, and staff, has compounded the problem.<\/p>\n

Patients in Wales and Northern Ireland face additional hurdles, as neither nation has its own heart or lung transplant centre, forcing them to travel to England for surgery. Scotland has only one such centre.<\/p>\n

Waiting Times That Vary Wildly<\/h3>\n

The disparity in care is stark. Waiting times for a donated heart can range from eight months to four-and-a-half years, depending on which hospital a patient is referred to. Professor Manas said NHS England had “never actively managed” these waiting lists and had “seldom” attempted to make the system fairer.<\/p>\n

This inconsistency extends to staffing. Half of the UK’s six heart and lung transplant centres have lost their most senior surgeon in the past two years, with many medics seeking better opportunities abroad. Professor Manas attributed this exodus to “inadequate” workforce planning, which has led to “burnout among transplant surgeons and teams, recruitment and retention difficulties [and] increasing reliance on goodwill.”<\/p>\n

He added: “We have a system that assumes the consultant workforce will absorb pressure indefinitely.”<\/p>\n

Calls for Radical Change<\/h2>\n

The problems within heart and lung transplantation were first formally identified in a 2024 report by international experts, commissioned by the previous Conservative government. NHS England was tasked with implementing the report’s recommendations, but surgeons and campaigners say progress has been painfully slow.<\/p>\n

One of those calling for a more radical overhaul is Dr Zubir Ahmed, a kidney transplant surgeon and former health minister who resigned from the government in May. He told the BBC that while some aspects of the transplant system are “world leading”\u2014such as being free at the point of use\u2014the current structure is failing patients.<\/p>\n

Dr Ahmed believes the planned abolition of NHS England presents a unique opportunity to give NHS Blood and Transplant direct commissioning powers. This would allow patients to be moved swiftly between centres to receive the organs they need, rather than being tied to a single hospital’s capacity.<\/p>\n

“I don’t think there is ever a need to cancel a transplant because of capacity issues,” he said.<\/p>\n

The Human Cost of Delays<\/h3>\n

For patients like those waiting for a new heart or lungs, every cancelled operation and every year on a waiting list can be a matter of life or death. The emotional and physical toll is immense, and the system’s failures are not just bureaucratic\u2014they have real, human consequences.<\/p>\n

The BBC has previously reported on individuals who have waited years for a transplant, their lives on hold while their conditions deteriorate. The frustration among clinicians is palpable, as they watch organs go unused or operations fall through due to systemic inefficiencies.<\/p>\n

NHS England’s Response<\/h2>\n

In response to the criticism, an NHS England spokesperson said: “Transplant services are complex and impacted by many factors, including the number of organs made available for transplantation, but the NHS is working hard to improve these lifesaving services.”<\/p>\n

They added: “NHS England is leading a programme of work which is already helping to increase the number of transplants, reduce variation, identify suitable patients earlier and improve access.”<\/p>\n

However, for Professor Manas and many others on the front line, these assurances ring hollow. They argue that without fundamental changes to how transplant services are commissioned and managed, the UK will continue to fall behind, and patients will continue to suffer.<\/p>\n

What Needs to Change?<\/h3>\n

Experts and clinicians have put forward several key recommendations to address the crisis:<\/p>\n

  • Give NHS Blood and Transplant the authority to commission transplant services directly, allowing for more flexible and responsive care.<\/li>\n
  • Invest in modern technology and equipment to increase the viability of donated organs.<\/li>\n
  • Expand intensive care and theatre capacity at transplant centres.<\/li>\n
  • Improve workforce planning to recruit and retain top surgical talent.<\/li>\n
  • Create a national oversight system to monitor and equalise waiting lists across all centres.<\/li>\n<\/ul>\n
